This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The Registered Nurse (RN) (Clinical Assessor) requires expert knowledge and understanding of the services and benefits available for Veterans and caregivers from both federal, state and private sector resources. The Clinical Assessor participates in face-to-face or virtual contact with the caregiver and Veteran to assess function and the need for personal care services. As a member of the interdisciplinary team, they will participate in the development and application of directed clinical treatment interventions. The incumbent will function in support of caregivers, Veterans and family members throughout the medical center and in community settings. The incumbent will conduct initial and annual home visits to Family Caregivers approved in the program and will conduct the Veteran Functional Assessment Instrument (VFAI) and other assessments as needed via phone and clinical video telehealth. The incumbent is responsible for developing and establishing linkages and collaborate on VA caregiver support efforts with community healthcare and social service agencies. The incumbent possesses expert knowledge and understanding of the services and benefits available for caregivers from both federal, state and private sector resources. The incumbent demonstrates expertise in practice based on conscious, deliberate planning. Self- directed in goal setting for managing complex patient situations.
