The Consolidated Call Center RN, is a professional registered nurse with a variety of experiences in ambulatory care nursing. The PCCC RN provides evidence-based care and functions as a coordinator of patient care within Primary Care in collaboration with the health care team. · Assesses, plans, implements, coordinates and evaluates the care of patients enrolled in the Primary Care who have complex physical and psychosocial needs. · Uses advanced clinical, interpersonal, and communication skills to assist patients/families to use services appropriately, to enhance the continuity of care and to achieve desired patient outcomes. · Incorporates the age, gender, cultural and learning needs of patients · Applies infection control measures appropriately in delivering care · Adheres to ethical principles and nursing standards in delivering care · Demonstrates use of effective skills for communication and patient education with individuals and groups · Demonstrates mandatory organizational and clinical competencies of the position.
